Look at the way the question is phrased: "Have you ever undergone any electric, insulin or other shock treatment or psychiatric brain operation?" If expanded, that would read: "Have you ever undergone any electric shock treatment, insulin shock treatment or other shock treatment or psychiatric brain operation?" Although it's mostly forgotten about today, insulin shock treatment was at one time used for many of the same disorders as electric shock treatment. I'm not sure of the reasons it fell out of favor (and the Wikipedia article, predictably, has a Scilon 'it was only ever used because of the cruelty of psychs' whiff about it, so I'm not about to trust their account) but for whatever reason, it stopped being done and now most people when they hear "shock treatment" are unaware that there ever was any type other than electric.
